Waiver Coordinator (Two Positions Available) - NY Association on Independent Living

The New York Association on Independent Living (NYAIL) is seeking two Waiver Coordinators to assist individuals transitioning from institutional settings to the community. The Open Doors Transition Center project is a NYAIL program funded by the NYS Department of Health’s Money Follows the Person project, which assures that residents of long-term care facilities are provided objective information and given the opportunity to live in a community-based setting.

NYAIL is a state-wide, non-profit organization, working with Independent Living Centers across the State to eliminate physical, communications, attitudinal, and other barriers for people with disabilities. NYAIL members are Independent Living Centers (ILCs), consumer controlled, cross-disability providers of advocacy, services and supports to people with disabilities.

 

Responsibilities

The Waiver Coordinators will report to the Lead Waiver Coordinator. They will be responsible for helping nursing home residents transition from nursing homes with the support of the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Waivers. Responsibilities include, but are not limited to:

  • Meet with participants interested in leaving nursing home with waiver services.
  • Work with nursing home staff, participant, family member and other team members to gather information needed to facilitate transition. Document all contacts and information gathered in the NYAIL database.
  • Develop relationships with Regional Resource Development Centers (RRDCs) and waiver service providers.
  • Work with service coordinators to ensure that a service plan is developed for assigned participants.
  • Work collaboratively with NYAIL’s Open Doors team.
  • Participate with Open Doors team to develop and provide training for Transition Specialists, and Peers, including but not limited to: annual trainings, NYAIL conference, regional trainings and one-on-one transition specialist trainings.
  • Enter referrals received via phone, email or fax into the database.
  • Answer calls to the 800 or NYAIL main number about Open Doors.
  • Meet regularly with NYS Department of Health & OPWDD staff to exchange information and follow-up on requested items to ensure all participant information is accurate and up to date.
  • Perform other duties as assigned.

Qualifications

  • Bachelor’s degree preferred.
  • Experience with Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Waivers required.
  • Service Coordination experience writing Initial Service plans strongly preferred.
  • Experience with the concepts of Independent Living, consumer control, and a strong commitment to promote and advocate for the full integration of individuals with disabilities.
  • Knowledge of the state’s Medicaid program and community resources that assist people with disabilities in living independently.
  • Excellent interpersonal, organizational, computer, and communication skills.
  • Personal experience with disability strongly preferred.

 

Salary and Benefits:

NYAIL offers a competitive salary and generous benefits package. The salary for this position is in the range of $60,000 per year.

Location and Travel:

This is a primarily remote position.  Travel to NYC for meetings with participants and service providers will be required. Occasional travel to Albany will be required. Travel expenses will be reimbursed by NYAIL.
